Brake Failure

I recently had the bad experience of having my brake pedal hit the floor while in traffic. As you might guess I did end up rear ending the car in front of me. At first I thought it might be a line failed, but all the fluid was accounted for. Second was a inspection inside the car, and there was a problem. The bolted shaft that goes from the pedal into the booster had failed just beyond the lock nut. It just sheered completely off. Nothing is binding, everything is free moving. Have you heard of this happening before or is this something I should concern myself with when dealing with all my older cars? After all its a spot one never thinks to inspect.
