Camshaft timing codes coming up

Vehicle Information: Year: 1997; Engine (type, size, modifications): 2.5; Total Mileage: 53000;

Recently, the DME has been throwing codes for camshaft timing errors and camshaft drive chain out of position. It also is giving codes for oxygen sensors (which it was also doing before the camshaft codes started coming up) and codes for misfiring on 1-2-3 (since the camshaft codes started). The engine runs quietly and normally in the low rpm ranges, but bogs down under full throttle at engine speeds over 4000-5000 rpm. More recently, there is sometimes a very light metallic rattling noise heard over the driver's left shoulder under heavy acceleration north of 4000 rpm. The codes come back within a few minutes after clearing them.Could the timing chain really be out of position and how would it have gotten that way? Could this constellation of codes mean something else altogether?
