T O Bearing will not stay engaged with fork

Model: 911, Year:1971, Mileage:270000, Type of use:Street use only
901 transmission just had a complete rebuild, 7K miles on NOS aluminum PP clutch disk, new TO bearing replaced at same time.Worked fine for 7K until trans. went away. Cable is reasonable new and in good condition. Here is the problem, TO bearing will not stay put with fork, clocks in OK, clutch feels normal but start the engine and clutch peddle drops to floor and TO bearing spins off fork, adjustment about 1" pull back and over center would just be felt. Took things apart again and could not see any discrepancies other than maybe a little too much play on fork and ball, could this be the problem? Oh God, keeping an old car alive! What's your take? Thanks, Charlie
